Forms for persons in the petroleum activities
From 2014 the doctor must be a petroleum doctor approved by the County Governor. The certificate was revised in 2017.

Below you can download the forms issued by the Directorate of Health for use in the certification of persons in the petroleum activities. They can be filled out electronically, but they must be printed and signed in the usual way.
In the health certificate there is a space for the doctor's stamp. The Directorate of Health has determined that if the name, ID-number (pin), address and phone number are readily apparent with machine or written capital letters, it can replace the stamp.
